Your physical can be uploaded directly onto your Aktivate account. They do not need to be turned into the athletic office.
Carson City School District has implemented a new digital platform for students to travel to and/or from an athletic contest. Please click the link to access SmartWaiver
This form must be submitted 48 hours in advance the date of the athletic contest.
Approval is at the coach & administrator's discretion.

This must be filled out in order to participate in a sport off-season (e.g., open gym, pre-season practice, spring training, etc.).
Register My Athlete is now Aktivate. These instructions are for Register My Athlete but the platforms are virtually the same. This information will be update soon
This form outlines the additional responsibilities of Carson High School students. They represent Carson High School on AND off campus and are subject to consequences as a result of their actions.
​Please only submit the last page (the page signed by a physician). The rest are for your medical records are are considered private information.

This includes important policies, agreements, and information for parents and athletes.
This form is for off-season activities and transportation that is not being provided by the school district, relinquishing CCSD and it's subsidiaries from all liability in regards to an activity.

The district is not responsible for medical bills of students injured at school, however, Student Accident Insurance is available to reduce the financial burden on families and ensure students receive the necessary medical treatment.
